Laxyl is indicated in-
Emotional disturbances, i.e. acute tension and anxiety states. Difficulties in interpersonal contact. Agitation, insomnia, anxious and agitated depressive reactions.
Functional disturbances in the cardiovascular and respiratory systems, i.e. pseudoangina pectoris, pericardial anxiety, tachycardia, emotiogenic hypertension, dyspnea and hyperventilation.

আবেগজনিত জটিলতায় যেমন-তীব্র মানসিক চাপ ও উদ্বেগ, পারস্পরিক সম্পর্কের অবনতি, অস্থিরতা, অনিদ্রা, অস্থিরতার কারণে সৃষ্ট বিষন্নতা।
হৃদযন্ত্র ও শ্বাসতন্ত্রের জটিলতায় যেমন- সিউডোএনজাইনা পেকটোরিস, পেরিকার্ডিয়াল এংজাইটি, ট্যাকিকার্ডিয়া, আবেগজনিত উচ্চ রক্তচাপ, ডিস্প্নিয়া এবং হাইপার ভেন্টিলেশন।
বিভিন্ন দীর্ঘস্থায়ী রোগের কারণে সৃষ্ট মানসিক উদ্বেগ নিরসনে নির্দেশিত।
🔹Pharmacologyফার্মাকোলজি
Bromazepam is a powerful psychotropic agent. In lower dosage, it selectively reduces tension and anxiety. In higher dosage, it shows sedative and muscle-relaxant properties. Bromazepam binds to the GABA-A receptor producing a conformational change and potentiating its inhibitory effects. Other neurotransmitters are not influenced.
ব্রোমাজিপাম মানসিক যন্ত্রনায় নির্দেশিত অত্যন্ত কার্যকরী ঔষধ। স্বল্প মাত্রায় সেবনে এটি ক্ষেত্রবিশেষ দুশ্চিন্তা এবং মানসিক চাপ কমায়। অধিকমাত্রায় সেবনে এটি সিডেটিভ বৈশিষ্ট্য ও মাংসপেশীর শিথিলতা উদ্রেক করে।
🔹Dosage & Administrationমাত্রা ও ব্যবহারবিধি
Standard dosage: Average dosage for outpatient therapy is 1.5-3 mg up to three times daily. Treatment of outpatients should begin with low doses, gradually increasing to the optimum level.
In severe cases,especially in hospital: 6-12 mg 2 or 3 times daily. The overall treatment generally should not be more than 8-12 weeks. In certain cases extension beyond the maximum treatment period may be necessary; if so, it should be taken with re-evaluation of the patient's status with special expertise.
Elderly and debilitated patients: Elderly patients and those with impaired hepatic functions require lower doses.
Children: Bromazepam is usually not indicated in children, but if the physician feels bromazepam treatment is appropriate, then the dose should be adjusted to their low bodyweight (about 0.1-0.3 mg/kg bodyweight)
স্বাভাবিক মাত্রা: হাসপাতাল বহির্ভূত রোগীর ক্ষেত্রে ১.৫-৩ মিঃগ্রাঃ করে দৈনিক তিনবার পর্যন্ত। হাসপাতাল বহির্ভূত রোগীর ক্ষেত্রে ব্রোমাজিপাম স্বল্পমাত্রায় সেবন শুরু করতে হবে এবং ধীরে ধীরে এর মাত্রা বাড়ানো যেতে পারে।
মারাত্মক অবস্থায়, বিশেষত হসপিটালাইজড রোগীদের ক্ষেত্রে: ৬-১২ মিঃগ্রাঃ করে দৈনিক দুই থেকে তিনবার। তবে নির্দেশনার সময়কাল ৮-১২ সপ্তাহের অধিক হওয়া উচিৎ নয়। কোন কোন ক্ষেত্রে বেশী সময় ধরে চিকিৎসার প্রয়োজন হতে পারে। তবে সে ক্ষেত্রে রোগীর অবস্থা বিশেষজ্ঞের দ্বারা ভালোভাবে পর্যবেক্ষণ করতে হবে।
বয়োঃবৃদ্ধ ও দুর্বল রোগী: বয়স্ক এবং যকৃতের কার্যক্ষমতা সীমিত এমন রোগীর ক্ষেত্রে স্বল্প মাত্রায় ব্রোমাজিপাম সেবন করতে হবে।
শিশু: ব্রোমাজিপাম শিশুদের ক্ষেত্রে নির্দেশিত নয়। তবে চিকিৎসকের পরামর্শ অনুযায়ী বিশেষ প্রয়োজনে সেবন করানো যেতে পারে। এক্ষেত্রে ঔষধ সেবন মাত্রা শিশুর দৈহিক ওজনের (০.১-০.৩ মিঃগ্রাঃ/কেজি দৈহিক ওজন) উপর ভিত্তি করে নির্ধারণ করতে হবে।
🔹Interactionমিথস্ক্রিয়া
If Laxyl is combined with other centrally active drugs, its sedative effects may be enhanced. These drugs are antidepressants, hypnotics, narcotics, antipsychotics, sedatives, antiepileptic drugs, sedative antihistamines and anesthetics. Co-administration of cimetidine may prolong the eliminiation half-life of Laxyl. Concomitant intake of Laxyl with alcohol should be avoided, because the sedative effect of Laxyl may be intensified by alcohol.
কেন্দ্রীয় স্নায়ুতন্ত্রে ক্রিয়াশীল ঔষধের সাথে ল্যাক্সিল ব্যবহার করলে এটির সিডেটিভ কার্যকারিতা বৃদ্ধি পেতে পারে। এইসব ঔষধের মধ্যে রয়েছে এন্টিডিপ্রেসেন্টস, হিপনোটিক্স, নারকোটিক্স, এন্টিসাইকোটিক্স, সিডেটিভ্স, এন্টিইপিলেপটিক্স, সিডেটিভ এন্টিহিস্টামিন্স এবং এনেসথেটিক্স। ল্যাক্সিলের সাথে সিমেটিডিন পাশাপাশি সেবন করলে ল্যাক্সিলের অপনয়ন অর্ধজীবন বৃদ্ধি পাবার সম্ভাবনা থাকে। অবশ্যই এলকোহল এর সাথে ল্যাক্সিল সেবন বন্ধ করতে হবে, কারণ ল্যাক্সিল এলকোহলের সাথে সেবন করলে তন্দ্রাচ্ছন্নতার মাত্রা বাড়তে পারে।
🔹Contraindicationsপ্রতিনির্দেশনা
Bromazepam is contraindicated in patients with known hypersensitivity to bromazepam, severe respiratory insufficiency, severe hepatic insufficiency or sleep apnea syndrome.
ব্রোমাজিপামের প্রতি অতিসংবেদনশীল রোগীদের ক্ষেত্রে, যকৃত ও শ্বাসতন্ত্রের মারাত্মক কার্যক্ষমতা হ্রাস, স্লিপ এপনিয়া সিনড্রোম এর ক্ষেত্রে ব্রোমাজিপাম প্রতিনির্দেশিত।
🔹Side Effectsপার্শ্ব প্রতিক্রিয়া
Common side-effects include fatigue, drowsiness, muscle weakness, numbed muscle, reduced alertness, confusion, headache, ataxia etc. These phenomena occur predominantly at the start of therapy and usually disappear with prolonged administration. Anterograde amnesia may occur using therapeutic doses.
সাধারণ পার্শ্ব প্রতিক্রিয়াগুলো হচ্ছে অবসাদ, ঝিমুনি, মাংসপেশীর দুর্বলতা, পেশীর অসঙ্গতি, অসতর্কতা, বিভ্রান্তি, মাথাব্যথা, এটাক্সিয়া ইত্যাদি। এই প্রতিক্রিয়াগুলো চিকিৎসা শুরুর প্রথমদিকে হতে পারে এবং ঔষধ সেবন চালিয়ে যাওয়ার ফলে এইসব প্রতিক্রিয়া থেকে মুক্ত হওয়া যায়। এন্টারোগ্রেড অ্যামনেশিয়া অনেক সময় পরিলক্ষিত হতে পারে।
🔹Pregnancy & Lactationগর্ভাবস্থায় ও স্তন্যদানকালে
The safety of bromazepam during pregnancy has not been established. As bromazepam is excreted in breast milk, use should be avoided during lactation.
গর্ভাবস্থায় ব্রোমাজিপাম ব্যবহারের নিশ্চয়তা প্রতিষ্ঠিত হয়নি। যেহেতু ব্রোমাজিপাম মাতৃদুগ্ধে নিঃসরিত হয়, সেহেতু স্তন্যদাত্রী মায়েদের ক্ষেত্রে এটি ব্যবহারে বিরত থাকা উচিত।
🔹Precautions & Warningsসতর্কতা
The use of benzodiazepines and benzodiazepine like agents may lead to the development of physical and psychological dependence upon these products. This dependence depends on the dose and duration of treatment; it is also greater in predisposed patients with a history of alcohol. Once physical dependence has developed, termination of the treatment will be accompanied by withdrawal symptoms. These may consist of headache, muscle pain, extreme anxiety, tension, confusion and irritability. Since the risk of withdrawal phenomena and rebound phenomena is greater after abrupt discontinuation of the treatment, it is recommended that the dosage be decreased gradually. Laxyl is not recommended for the primary treatment of sleeplessness caused by psychotic illness. Caution should be exercised while driving cars or using machineries.
বেনজোডায়াজিপিন এবং বেনজোডায়াজিপিন এর মত এজেন্ট সেবনের ফলে এর প্রতি রোগীর দৈহিক ও মানসিক নির্ভরতা জন্মে। এই নির্ভরতা ঔষধের মাত্রা ও সেবনকালীন সময়ের উপর নির্ভর করে। এলকোহলসেবীদের ক্ষেত্রে এই নির্ভরতা বেশী দেখা যায়। এই ঔষধটি হঠাৎ করে সেবন বন্ধ করার ফলে প্রত্যাহার পরবর্তী অবাঞ্ছিত প্রতিক্রিয়া যেমনঃ মাথাব্যথা, পেশীতে ব্যথা, তীব্র উদ্বেগ, অস্থিরতা, বিভ্রান্তি, ক্রোধ প্রবণতা পরিলক্ষিত হয়। তাই ঔষধ প্রত্যাহার পরবর্তী অবাঞ্ছিত প্রতিক্রিয়া এড়ানোর জন্য ঔষধ সেবনের মাত্রা ধীরে ধীরে কমিয়ে, ঔষধ সেবন বন্ধ করতে হবে। মানসিক সমস্যাজনিত অনিদ্রার ক্ষেত্রে প্রাথমিক চিকিৎসা হিসেবে ল্যাক্সিল নির্দেশিত নয়। গাড়ী চালানোর সময় বা ভারী যন্ত্রপাতি চালানোর সময় সতর্কতা অবলম্বন করা উচিৎ।
🔹Storage Conditionsসংরক্ষণ
Keep in a dry place away from light and heat. Keep out of the reach of children.
